ENDOCRINOLOGY (HORMONES),


Endocrinology is a speciality that deals with disorders of various endocrine glands. All the hormonal tests required for evaluating endocrine disorders are available at Jupiter Hospital. The disorders of adults are looked after by an adult endocrinologist and that of paediatric age by a paediatric endocrinologist. The disorders treated under this branch are given below.

 

Diabetes Mellitus

India is considered the world capital of Diabetes. Hence, this disease management requires an experienced physician with dietician to get the disease controlled as well as prevented.


The management of Diabetes includes nutritional counselling and during pregnancy including gestational diabetes. Nutritional counselling is given by a registered nutritionist to take care of the individuals needs and food preferences, which is necessary for compliance and so is a balanced diet to prevent vitamin deficiencies. The aim of endocrinologist is to empower patients to manage their own diabetes especially when they are on insulin. The patients are taught to adjust the dose of insulin on the basis of their home glucose monitoring by glucometer.

In case of severe obesity, the hospital also has a facility for Bariatric Surgery if required; however, this is what the endocrinologist tries to prevent.

Thyroid

Thyroid disorders are as common as diabetes, but they can be easily managed than diabetes. The diseases consist of goiter, hypothyroidism, hyperthyroidism and thyroid nodules. Hypothyroidism in young age can cause growth failure (body and brain) and hyperthyroidism can affect the heart and bones. Hence, these disorders require expert management.

Also thyroid disorders in pregnancy can affect the mother as well as foetus and the necessary care will need an endocrinologist.

The surgical facility for thyroid if needed is available for conditions like very large glands, malignancy and sometimes in thyrotoxicosis.



Metabolic bone disorders

This consists of rickets, osteomalacia, osteoporosis and parathyroid disorders (low and high calcium levels). Vitamin D deficiency is very common and has implications on fractures in later life. Today's life style and pollution contribute to D deficiency and hence need management.



Parathyroid disorders

Ovarian failure, polycystic ovaries, delayed, early or absent puberty, idiopathic hirsutism and others can be managed. Polycystic ovaries occur in individuals with family history of diabetes and is considered to develop diabetes in later life. Testicular disorders

This includes hypogonadism and delayed puberty.



Adrenal disorders

Deficiency of cortisol secretion causes adrenal insufficiency which if not recognized can cause even death. The management requires a trained endocrinologist to prevent too much versus too little steroids.

The excess of cortisol causes obesity and hypertension. The investigation of this disorder is demanding and requires experience, as this disease is potentially fatal.

The adrenal gland also secretes certain hormones causing hypertension, these are usually tumours of adrenal medulla pheochromocytoma and hyperaldosteronisml hypertension. We have been managing these disorders at Jupiter Hospital, including performing surgery when required.



Pituitary disorders

Pituitary is considered the bandmaster of all endocrine secretions except insulin and parathyroid hormone. The deficiency produces short stature, low thyroid hormones low adrenal hormones and loss of periods, we call this pan hypopituitarism. The treatment may be lifelong and requires management by endocrinologists proper hormone replacement. Pituitary also secretes excess hormones at time prolactinoma (prolacting secreting tumours) and Cushings syndrome (ACTH secreting pituitary tumour causing excess adrenal steroids).
